NEXUS SPECIALTY HOSPITAL OPENS WOUND CARE CLINIC IN SHENANDOAH, TEXAS
November 30, 2011
Nexus Specialty Hospital is expanding its nationally recognized wound care program with the opening of Nexus Specialty Clinic, a hospital-based wound care clinic in Shenandoah, Texas. Nexus Specialty Clinic will offer comprehensive, state-of-the-art wound care treatment and therapy for out-patients with complicated, non-healing wounds and chronic illness.
Nexus Specialty Clinic specializes in the treatment of the following wound types: pressure ulcers, neuropathic diabetic foot ulcers, surgical wounds, traumatic wounds, arterial and venous wounds, and compromised skin and/or wound conditions. The Clinic's outpatient wound care clinic is an extension of Nexus Specialty Hospital's successful inpatient wound management program. With a facility healing percentage of 98 percent, Nexus' evidence-based wound care protocol has been nationally recognized with a Gold Seal of Approval by the Joint Commission as well as recognized as noteworthy by the DNV.
Nexus Specialty Clinic will be led by an interdisciplinary team of wound care physicians who are board-certified in Family Medicine, Physical Medicine and Rehabilitation, Osteopathic Medicine, Podiatry, and Wound Care. Services provided include surgical debridement, pulsatile lavage, autologous platelet grafting, application of bioengineered skin substitutes, negative pressure wound therapy, and patient/ caregiver education.
"Nexus Specialty Clinic fills a void in the community by offering a continuum of services that emphasizes an interdisciplinary team model backed by a proven, evidence-based wound management program," explained Cindy Mostaffa, Nexus Specialty Hospital CEO.
"We opened Nexus Specialty Clinic in response to community demand. Nexus Specialty Hospital is known for its specialized and procedural-based wound care, innovative treatment protocols, and improved healing rates. Nexus uses advanced therapies and proven clinical protocol to successfully treat complex wounds that have previously resisted healing. Unfortunately, we noticed that patients would be discharged from Nexus Specialty Hospital healing well, only to end up back in the hospital months later due to lack of or inadequate follow-up care," Mostaffa disclosed.
"Nexus Specialty Clinic is designed to address this issue and serve as a referral resource for community physicians. After wound patients are discharged from the hospital, they can continue to receive the same level of high-quality wound treatment and therapy that Nexus is known in an outpatient setting. Local physicians can refer their chronic cases to us, and together we can develop a treatment plan that circumvents debilitating health problems and prevents life-threatening infections. The ultimate goal is to minimize hospital admissions by decreasing wound reoccurrence," Mostaffa continued.
"When Nexus Specialty Clinic patients require advanced support, our staff physicians can readily leverage all the benefits of Nexus Specialty Hospital -- dieticians, infections disease specialists, , neurologists, pulmonologists, physical, occupational and speech therapists, radiologists, plastic and vascular surgeons, and state-of-the-art imaging technology -- ensuring a seamless continuity of care and improved patient outcomes," Mostaffa concluded.
As a valued added service, Nexus Specialty Clinic will also provide spinal cord injury consultations, post-acute follow up, and annual assessments. Patients diagnosed with spinal cord injury (SCI), brain injury, stroke, spina bifida, and cerebral palsy are at an increased risk for pressure ulcers and other wounds.
Located in a newly remodeled facility at 114 Vision Park Blvd., Nexus Specialty Clinic will feature dedicated exam and treatment rooms designed to accommodate bariatric patients and their family members in a friendly, compassionate, and welcoming setting. Hours are Monday - Friday, 8:30 - 5:00. For more information or to refer patients, call 713-482-3390.

November 2011
Nexus Health Systems, a provider of post-acute medical care, transitional and vocational rehabilitation services, was recently named to Houston Business Journal's list of the largest hospitals in the region. Nexus Health Systems placed #13 on the list, which was ranked by number of operating beds. This is Nexus' second appearance on the list.
"Winning this award exemplifies Nexus' core values, exemplary patient care, and ability to maximize the utilization of our resources. And while being named to the list is an honor, it is important to remember what this award recognizes," John W. Cassidy, MD, President, CEO and Chief Medical Officer for Nexus Health Systems explained. "It's really about the quality of care that our medical staff, employees and volunteers have provided to our patients and their families."
“Nexus Health Systems offers the country's only continuum of care for neurologically injured patients. This ensures better care and better outcomes for our patients," Cassidy said. "As a physician-led, patient-centered, family focused system, our philosophy is to always to place the patient's needs first and assure the people in our community who face neurological injury or chronic illness receive the best care possible."
"With the opening of Nexus Specialty Clinic, a hospital-based wound outpatient care facility in Shenandoah, Texas, Nexus Health Systems is poised for continued growth in 2011. We look forward to continuing to serve the patients, physicians, and families who depend on us for specialized and procedural-based care, advanced therapies, and innovative treatment protocols," Cassidy concluded.

Touchstone Neurorecovery Center Awarded Brain Injury Contract by Dept of Veterans Affairs
August 2011
The only facility in Houston area that received the contract and among a couple dozen facilities across the nation that received a contract
Touchstone Neurorecovery Center in Conroe is among one of only about 25 facilities in the nation, and the only in the Houston-area, to be awarded a contract by a U.S. Department of Veterans Affairs to provide veterans with comprehensive brain injury rehabilitation services. Through the Assisted Living for Veterans with Traumatic Brain Injury (AL-TBI) Pilot Program, the VA provides selected private civilian health care entities with funding to deliver specialized recovery support in a more home-like environment.
The AL-TBI program was established through the National Defense Authorization Act of 2008 and allows the VA to give veterans an alternative to hospitalization or institutional care and deliver services in a more efficient and cost-effective manner.
"The entire Touchstone team is proud to be selected through this program to offer veterans with brain injuries a full continuum of rehabilitation services and help them reclaim the greatest level of independence and quality of life possible," said John Cassidy, M.D. “Now veterans’ families in Houston and across Texas can arrange for medical, physical, behavioral, vocational, and community re-entry care for their loved one, in a comfortable setting much closer to home."
The AL-TBI pilot program also is designed to help the Defense and Veterans Brain Injury Center study the effectiveness of post-acute rehabilitation in the outcomes of brain injury patients. As such, Touchstone will be collecting information about specialized rehabilitation treatment protocols to enhance understanding of the impact different approaches have on patient outcomes.
"We believe that this research has the potential to advance medical care of brain injury patients in a significant way, and we are excited that Touchstone Neurorecovery Center will get to play a significant role in that process," said Dr. Cassidy.

HealthBridge Children's Hospital of Houston now using WeeFIM Outcomes
July 2011
HealthBridge Children's Hospital of Houston is proud to announce that they are now participating in Outcome Measurement information gathering using the WeeFIM II System. This information will be gathered and then reported on quarterly demonstrating outcomes for their patient populations.
- The WeeFIM II System allows credentialed clinicians across the United States and around the world to measure and document functional performance in infants (0-3 years old), children, and adolescents with either acquired or congenital disease in a consistent manner. The WeeFIM instrument is a reliable outcomes measurement instrument that can be applied uniformly across inpatient, outpatient, and community-based settings to track clinical, managerial, and performance improvement efforts and initiatives. The WeeFIM II System is a benchmarked outcomes management system that provides a method of evaluating outcomes for patients, groups of patients (population-based), and overall medical rehabilitation/habilitation programs.
-
The WeeFIM® instrument was developed to measure the need for assistance and the severity of disability in children between the ages of 6 months and 7 years. The WeeFIM instrument may be used with children above the age of 7 years as long as their functional abilities, as measured by the WeeFIM instrument, are below those expected of children aged 7 who do not have disabilities. The WeeFIM instrument consists of a minimal data set of 18 items that measure functional performance in three domains: self-care, mobility, and cognition. The WeeFIM® Instrument: 0-3 Module is a questionnaire that measures precursors to function in children 0-3 years old who have a variety of disabilities. The 0-3 module can be administered to parents by interview or self-report and is useful across many settings, including early intervention and preschool.
Baylor at HealthBridge

Joseph Rafferty 713-351-6622
HealthBridge Children's Hospital is pleased to announce a new partnership with Baylor College of Medicine, in which physicians will teach residents a team-based, inter-disciplinary approach to caring for children with chronic needs.
"Our residents already learn what it's like to take care of acute problems, but what happens if the child requires chronic care that can't be managed in the home?" said Dr. Teri Turner, associate professor of pediatrics and associate director of house-staff education at Baylor.
They go to HealthBridge, a facility for children leaving acute care facilities but who aren't quite ready to transition, or cross the bridge, home.
The 40-bed, west Houston hospital has been providing comprehensive, interdisciplinary care for medically-complex pediatric patients in a home-like, non-institutional setting since 1999.
Its team-based approach is something Baylor residents are eager to learn.
"Functioning as a team is not traditionally how we've been trained in medicine, so this is a fairly new arena," Turner said. "A lot of communication has to take place. It's 'teach and learn.' Doctors are used to the teaching, but not so much the learning from other health professionals. That's why this is a great partnership."
There are 48 professionals on staff at HealthBridge, including pediatricians, physical therapists, psychiatrists, nurses, dieticians, nutritionists, child life specialists, social workers and child educators. In 2006, the University of Texas Medical School Pediatric Residents (UT) began rotating at HealthBridge and have continued this practice through present day.
"That is where the art of medicine comes in. We are all involved in patient care," said HealthBridge Pediatrician Dr. Jill Ann Jarrell. "It's a different approach, but now healthcare reform demands more efficient care of complicated cases, not just in adults, but children too."
HealthBridge has been doing just that for 12 years, and now Turner is looking forward to Baylor getting on board.
"It's no longer just the physician, or just the physician and nurse," Turner said. "That's simply not an efficient system. Instead, everyone adds to the care of the children, who are living longer with chronic diseases. We must learn how to maximize care and let them live life to the fullest."
Approximately 45 first-year Baylor residents will be on the HealthBridge rotation each year, three at a time.
They will learn about pulmonary medicine for infants and children with neurological injuries or breathing difficulties; therapeutic infusion for children who require hydration, antibiotic management, nutrition, intravenous pain management, transfusions and immunotherapy; gastroenterology for children and their families to support new feeding skills; and physical medicine/rehabilitation for children who have lost the ability to walk, talk, eat or breathe due to injury or illness.
"My goal is to have them understand the scope of what we do at HealthBridge, how we take care of a pretty sick child with a complicated case," said Jarrell, who will invite residents to shadow her as she sees patients.
The relationship is turning out to be a win-win for all.
"The residents are brilliant and hearing their ideas is exciting," said Jarrell, herself a Baylor College of Medicine graduate.
"This is a very big deal when it comes to pediatric healthcare in Houston," said HealthBridge Chief Executive Officer Joseph Rafferty. "When the community hears that the UT and Baylor Pediatric Residents are here, it's a stamp or seal of approval that we are providing amazing care. Baylor’s presence complements the great care we are already providing at HealthBridge."
